Baseball Umpire Forms
All Baseball Umpires will be required to fill out a background check. All background checks can be mailed in to: KYSA Secretary, at P.O. Box 20033 Keizer, Oregon 97303. Baseball Umpires who are paid will recieve all payments by check this year. Paid Umpires will be required to fill out an I-9 and a W-4.
